Beginning Sept. 1, 2022, the Tribunal is using new intake forms for formal marriage nullity cases. Presenting a case using the new forms requires working with one of our regional advocates, who are listed for each group of parishes on the form below. If you have not received training in the use of the new intake forms, please contact the Tribunal or your regional advocate for help in getting acquainted with the process. The vademecum, which is linked below, also explains how to use the new forms as a case counselor. Please submit a case using the old form only if it was already begun using that form.
