Notes:

• Pastor support is required in this application.
• Completion of this application does not assure acceptance of the applicant to the program. A decision notification will be delivered between Oct. 26 to Nov. 3.
• Applicants accepted to the program should honor the commitment through the time of the initiative. This includes attending an in-person ceremony on Nov. 6, in-person prayer and fellowship on Dec. 4, and in-person and online gatherings as listed in this calendar.
